We have been collaborators with community-based organizations such as United Way 211 in designing and implementing unique benefit programs to address social needs for members across the lifespan and life experience. Offerings include transportation benefit services; home-delivered meals after an inpatient hospital, observation, or skilled nursing facility stay; and a home stabilization program that provides resources to members to resolve unsafe housing issues so that they can remain safely in their homes and avoid a nursing facility placement.
