Background

The Neighborhood Center (NC) was established in 2022 as a community-facing multi-purpose resource to address members’ and community social needs via services and Community Health Workers (CHWs), provide a free space and resources to community-based organizations with health-adjacent missions, and serve as a community-based location for a UPMC Health Plan Connect Center.

Healthcare Challenge

The NC is located in the nexus of several communities experiencing health inequities and social disinvestment.

Goals

The project aims to test and evaluate the outcomes associated with a new model of community-based services for members and the general community. Outcomes center around the building and nurturing of partnerships with community-based organizations (CBOs) whose missions align with UPMC Health Plan’s and the provision of a new access point for CHWs and the UPMC Health Plan Connect Center to meet with members to assess and address health and social needs and answer questions about insurance access.
